How do we decide if my resume needs to be changed based on application results?

Data-driven decisions only. Until you reach ~100 applications, don't assume it's a resume problem. At 200+ with no callbacks, your CSM will investigate — looking at timing of application (were you in the first 24 hours?), which resume version was used, and where in the funnel you're dropping off.
